BECOME A MEMBER OR RENEW YOUR EXISTING MEMBERSHIP
(YOU CAN PAY FOR A NEW MEMBERSHIP OR RENEWAL OF YOUR EXISTING MEMBERSHIP, BY ETRANSFER.)
You can either download the Application Form below or use the fillable function online. Just click on the image and it will open in a separate tab.
1. DOWNLOAD:
If you download the form, you can print it out, complete it , scan it and either email it as an attachment to treasurerclwa@gmail.com . Or you can 'snail-mail' it to the P.O. Box address below.
OR...you can open the file in Adobe Reader (or a similar program) and fill-in your information. Once completed, you can save the file as a PDF, using a different filename. THEN...email the file as an attachment to treasurerclwa@gmail.com.
2. ONLINE:
Click on the image below. You can then fill in the form online (for your signature, just use your name) and save it to your device as a PDF file with different filename. As described in (1), email the file as an attachment to treasurerclwa@gmail.com
In your covering email, tell us that you will be sending your payment by E-Transfer, and include the password that you will use for the transfer process. Go to your online banking and make an etransfer to 
treasurerclwa@gmail.com. Be sure to use the same password that you gave us in the email, AND be sure to include your name and address in the message box in the transfer process, so that CLWA's Treasurer can match your payment to your Application Form.
AGAIN, HERE ARE TWO WAYS THAT YOU CAN GET YOUR COMPLETED APPLICATION TO CLWA:
1. Email it to treasurerclwa@gmail.com telling us that you will be paying by E-Transfer, provide the password you will be using and then go to your bank website and make your payment by E-Transfer to treasurerclwa@gmail.com
2. Mail it to CLWA at: P.O. Box 192
Marmora, ON K0K 2M0
....enclosing your cheque with it, if you are NOT paying by etransfer.
